1. Letter Application (Please indicate the Position and the Place of Assignment)
2. Updated Personal Data Sheet (PDS) CS Form No. 212 Revised 2017
3. CS Form No. 212 Work Experience Sheet
4. Authenticated Transcript of Records from the School Registrar
5. Authenticated copy of Certificate of Rating (CSC, PRC), if any
6. Authenticated copy of PRC Card (if applicable)
7. Certificate of Training/Seminar attended relevant to the position applied
8. Latest Performance Rating for two (2) rating periods (photocopy) if currently employed.
9. Authenticated copy of Good Standing (for RA 1080 eligibles only)

Only applicants with complete documents shall be considered for evaluation.
