2. Updated Personal Data Sheet (PDS) CS Form No. 212 Revised 2017
3. Authenticated Transcript of Records from the School Registrar
4. Original or authenticated copy of Certificate of Rating (CSC, PRC), if any
5. Certificate of Training/Seminar attended relevant to the position applied
6. CS Form No. 212 Work Experience Sheet
7. Latest Performance Rating for two (2) rating periods (photocopy) if currently employed.
8. Original or authenticated copy of Good Standing (for RA 1080 eligibles only)

Only applicants with complete documents shall be considered for evaluation.
