  4. Original or Authenticated Certificate of Rating (CSC, RA 1080), if any
  5. Certificate of Trainings/Seminar Attended relevant to the position applied (photocopy)
  6. CS Form No. 212 Work Experience Sheet
  7. Awards/Letter of Commendation received
  8. Latest Performance Rating for two (2) rating periods (photocopy) if currently employed
